Job summary

Curana Health is seeking a Care Ally, Case Manager to join our interdisciplinary care team. You will assess needs, plan care, coordinate services, and liaise with members, families, RP/POAs, and providers.

The role requires managing care plans, educating members and families, arranging appointments, coordinating with PCPs, and ensuring timely communication. Travel to facilities 2-3 days per week with hybrid work from home when not on-site.

Qualifications

• Minimum clinical practice in case management, home health, critical care, or discharge planning.
• Proficiency with basic computer tools (Word/Excel/Outlook) and multiple systems.
• RN/LPN license or social work degree with licensure; MSW preferred.

Responsibilities

• Execute on strategies to manage and improve member experience.
• Conduct comprehensive health assessments and develop care plans.
• Educate members and families about disease status, plan benefits, and resources.
• Coordinate with care team and PCPs to ensure appropriate treatment.
• Follow up with members to monitor progress and achieve health goals.
• Assist with appointments, transportation, and coordination of home health visits.
