Qualifications
- Bachelor’s Degree in Pharmacy or Doctor of Pharmacy Degree.
- Active Illinois clinical pharmacist license (IDFPR).
- Experience in specialty areas (neonatology, critical care, oncology) or completion of a hospital pharmacy residency is preferred.
Responsibilities
- Interprets medication orders to ensure safety and proper use of medications and coordinates problem-solving between pharmacy, nursing, and prescribers.
- Monitors therapy for significant drug interactions and intervenes as needed.
- Evaluates medication appropriateness and communicates potential problems and solutions to the prescriber.
- Provides medication information to healthcare professionals, patients, and the public and educates staff and learners.
Education
Bachelor’s Degree in Pharmacy or a Doctor of Pharmacy Degree
