Qualifications
- Bachelor’s Degree in Pharmacy or Doctor of Pharmacy required.
- Active Clinical Pharmacist license in the state of Illinois (IDFPR).
- Pharmacy residency or equivalent hospital pharmacy experience preferred.
Responsibilities
- Interprets medication orders to ensure safety and proper use of medications and coordinates with Pharmacy, Nursing, and prescribers.
- Monitors medication therapy for significant drug interactions and intervenes when needed.
- Evaluates and documents appropriateness of medications and communicates with prescribers about issues.
