Qualifications

• Bachelor’s Degree in Pharmacy or Doctor of Pharmacy (PharmD).
• Active Clinical Pharmacist license in Illinois (IDFPR).
• Pharmacy residency or hospital pharmacy experience (preferred).

Responsibilities

• Interprets medication orders to ensure safety and proper use; coordinates with pharmacy, nursing, and prescribers.
• Monitors medication therapy for significant interactions or therapeutic duplications; intervenes as needed.
• Evaluates and documents appropriateness of medications to achieve therapeutic outcomes.
• Educates patients and families about specific medications and provides clinical information to healthcare professionals.
• Maintains and efficiently uses the Electronic Health Record for medication history and documentation.
