Job summary

Qualifications

• High school diploma or equivalent is required.
• Minimum of two years of experience in related experience required.

Responsibilities

• Coordinate discharge and secure nursing and delivery with patient, referral source, sales, pharmacy, and nursing teams.
• Create, communicate and obtain all required documentation for new referrals.
• Assist patients in identifying payment options and financial assistance when needed.
• Support claims processes, follow up on paperwork, and assist with denial management.
• Assist Supervisor and Manager with special assignments as needed.
