Job summary

PneumaCare is seeking a County Operations Supervisor to oversee CalAIM ECM and Community Supports programs at the county level. You will supervise front-line staff, oversee case management, and plan outreach while coordinating with the MACC Benevolence team to ensure seamless service delivery.

You will drive quality improvement activities, participate in program meetings, and ensure compliance with HR policies and health plan requirements.

Qualifications

• Requires a degree in social work, sociology, human services or related field.
• Experience supervising clinical or social services teams with care coordination.
• Experience serving low-income populations with complex chronic conditions.
• Knowledge of CalAIM ECM/CS programs beneficial.

Responsibilities

• Supervise CalAIM ECM and CS county operations and frontline staff.
• Oversee case management systems and ensure quality service delivery.
• Coordinate with MACC Benevolence and partner agencies.
• Lead meetings, training, and performance evaluations for staff.
• Ensure compliance with health plan requirements and HR policies.
