Job summary

PneumaCare Health and Wellness is seeking a Community Care Coordinator to collaborate with the CalAIM ECM/CS programs. The role involves engaging members where they live, coordinating care plans, and ensuring seamless service delivery with MACC oversight.

The incumbent will facilitate assessments, update plans, arrange transportation, and work with providers to meet health and psychosocial needs. Travel to client sites is required.

Qualifications

• Experience in case management or social services is preferred.
• Ability to document clearly and maintain records.
• Comfort with multidisciplinary teamwork and reporting.

Responsibilities

• Coordinate Enhanced Care Management and Community Supports.
• Facilitate engagement, assessment, and follow-up with members.
• Develop and update care/housing plans with goals.
• Coordinate with service partners and transportation.
• Attend trainings and update staff on ECM templates.
• Travel to member homes and clinics as needed.
