Job summary

Simple Solutions Psychotherapy is hiring a Case Manager II (ECM Lead) to coordinate care for Medi-Cal members with complex needs under CalAIM. You will manage the care plan, lead the care team, and ensure seamless access to services across medical, behavioral health, and social systems.

The role requires an MSW preferred or a BA with 3+ years in case management, bilingual English/Spanish preferred, and a valid CA driver’s license.

Qualifications

• MSW preferred or BA in Social Work, Psychology, Nursing, Public Health or related field with 3+ years of progressive case management in Medi-Cal ECM or related programs.
• Bilingual English/Spanish preferred; CA driver license, reliable transportation and auto insurance required.
• ASW/AMFT/APCC registration a plus.

Responsibilities

• Lead CalAIM ECM Case Manager II with primary accountability for Care Plan, ICT coordination, and continuity of care.
• Conduct comprehensive biopsychosocial assessments and risk stratification; develop and revise Care Plans per DHCS guidance.
• Lead interdisciplinary care team meetings with primary care, behavioral health, CS providers, MCP care managers, and partners.
• Coordinate transitions of care from hospitals, SNFs, jails, and residential settings; ensure 30-day post-discharge follow-ups as required.
• Authorize, refer to, and monitor Community Supports; ensure documentation and MCP workflows.
• Mentor and provide case consultation to Case Manager I staff.
• Participate in quality improvement, case reviews, and fidelity monitoring of programs.
• Ensure documentation meets MCP audit standards and encounter data submission requirements.
