Job summary

Simple Solutions Psychotherapy is seeking a Lead Case Manager II for CalAIM ECM care coordination. You will manage a moderate-to-high acuity caseload, serve as primary lead for assigned members, and mentor junior staff while ensuring compliance with program standards.

You will collaborate with medical and behavioral health teams, coordinate services across agencies, and drive care plans that improve outcomes for Medi-Cal members. California driver’s license required.

Qualifications

• Master of Social Work (MSW) preferred, OR Bachelor's degree in Social Work, Psychology, Nursing, Public Health, or related field with 3+ years of progression in Medi-Cal ECM/care coordination.
• Bilingual (English/Spanish) preferred.
• ASW/AMFT/APCC registration a plus.
• Valid California driver’s license and auto insurance required.

Responsibilities

• Lead CalAIM ECM care management for a caseload of 30–50 members with complex needs.
• Coordinate with healthcare providers, CS providers, and community partners.
• Conduct biopsychosocial assessments and develop/revise Care Plans per DHCS ECM policy guide.
• Lead interdisciplinary care team meetings and coordinate transitions of care.
• Authorize and monitor Community Supports and ensure proper documentation for audits.
