Qualifications

• Two to three years of experience in care management, case management, healthcare navigation, or a related field.
• Bachelor’s degree in social work, public health, or a related field preferred.
• Experience navigating healthcare systems and public benefits (Medi-Cal, CalFresh, CalWORKs).
• Bilingual English and Spanish or English and Vietnamese highly preferred.
• Proficiency with Microsoft Office Suite and electronic documentation systems.

Responsibilities

• Provide direct care management and referral services for a caseload of ~30 members, including crisis intervention and health education.
• Conduct comprehensive assessments across medical, behavioral health, housing, and other social drivers of health.
• Develop and implement individualized, person-centered care plans in collaboration with members and providers.
• Coordinate care across healthcare providers, behavioral health services, and community organizations.
• Document assessments, care plans, and interactions in compliance with program requirements.
