Behavioral Health Services North Inc. in Plattsburgh, NY, seeks a Supervisor for the Breakthrough II Apartment Treatment Program to oversee daily operations, staff mentoring, and resident care in a 45-bed OMH licensed housing program.

You will coordinate admissions/discharges, ensure regulatory compliance, manage program finances, and collaborate with Nursing, Crisis, Finance and community partners to maximize funding and support residents toward independent living.

Qualifications

• Master’s and/or Bachelor’s degree in human services related field OR Associate’s degree in human services related field AND 3 years’ experience in human services OR High School Diploma AND 5 years’ experience in human services required
• Case Management and/or care coordination experience required
• Experience with crisis resolution and strong de-escalations skills required
• Strong interpersonal, communication, time management, problem solving, leadership, supervisory and organizational skills required
• Supervisory Management experience required
• 3 years’ experience with vulnerable population required; experience working with individuals recovering from severe mental illness and/or substance use disorders strongly preferred
• Proficient in Microsoft Office software such as MS Word & Excel are required
• Reliable transportation to transition between varying program sites required
• Must be able to lift up to 25 lbs and have the ability to climb and descend stairs
• Ability to work a flexible schedule to accommodate the needs of the program
• CPR and NARCAN certification required within 60 days of employment

Responsibilities

• Oversees the daily operations of the Apartment Program to ensure resident/program needs are met
• Facilitates, develops and assists with mentoring, on-boarding, training, supervising and monitoring Apartment Program staff and interns
• Establishes and coordinates staff schedule to ensure adequate coverage
• Coordinate care with mental health/substance use/primary care services, obtaining funding, transportation, linkage to other services, care coordination and discharge planning
• Coordinates and communicates admissions and discharges to the team
• Follow OMH and BHSN policies and regulations related to residential programs
• Participate in committees to screen Breakthrough II applicants for eligibility
• Oversees funding and Medicaid revenue; manages program finances
• Direct oversight of administrative staff and interns within Breakthrough II
• Collaborates with internal/external entities to coordinate residential and medical treatment
• Ensures monthly site safety checks and fire drills are completed and deficiencies resolved
• Collaborates with landlords to resolve disputes and review lease renewals
• Collects data (occupancy, surveys, reports) throughout the year
• Participates and ensures documentation of staff meetings
• Coordinates with Adult Residential Nursing to monitor medical management for the Apartment Program
• Ensures treatment team meetings are scheduled and outcomes communicated
• Provides/restorative services as indicated and documents timely per regulations
• Monitors QA activity and outcomes
• Observes self-administration of medications per protocol
• Maintains medication records with Adult Residential Nursing Team
• Reports facility issues to maintain a safe living environment
• Maintains communication with Compliance for incident reporting and program integrity
• Ensures service plans are followed and meet regulatory requirements
• Attends meetings with internal/external partners
• Performs other duties within scope
