Job summary

The Commonwealth of Pennsylvania, Wayne County Human Services, Behavioral Health & Intellectual Disabilities in Honesdale, PA, seeks a Caseworker 1 – Blended Case Manager to support children and families through needed services and community supports.

You will perform intake, coordinate services, assess needs, promote recovery, and monitor progress, with paid training and opportunities for advancement. Telework is not available.

Qualifications

• Two years of experience as a County Social Services Aide 3 plus 12 college credits in related social sciences.
• Bachelor's degree with 12 credits in sociology, social welfare, psychology, gerontology, or related fields.
• Any equivalent combination of experience and training with 12 credits in social sciences and one year of paraprofessional case management experience.
• Applicants may meet educational requirements within two months of graduating.

Responsibilities

• Intake support to enroll individuals in the Behavioral Health Blended Case Management Program.
• Link individuals with needed community services with providers and supports.
• Use Environmental Matrix to assess service needs on an ongoing basis.
• Promote resiliency and recovery following Behavioral Health principles.
