Pathways Forward in Harrisburg, PA is seeking a Mental Health Blended Case Manager to support individuals facing mental health challenges. You will locate, coordinate, and monitor essential services, develop individualized service plans, and advocate for clients within the human services system.

This full-time, day-shift role includes on-call responsibilities with rotating standby rate. Eligible for an incentive bonus up to $5,000 yearly, generous PTO, and comprehensive benefits.

Qualifikationen

• Bachelor’s degree with at least 12 college credits in sociology, social work, psychology, gerontology, criminal justice, or a related social science; OR
• Associate degree with at least 12 college credits in a relevant field AND two years of related experience.
• Valid driver’s license and automobile required.
• PA Child Abuse, PA Criminal History, and FBI clearances are mandatory.

• Inspire and motivate individuals to overcome barriers to recovery.
• Conduct strengths/needs assessments and create individualized Service Plans based on client preferences.
• Assist clients in accessing and securing necessary services using personal transportation as needed.
• Maintain and update Service Plans regularly to reflect client progress and needs.
• Build and sustain ongoing relationships with clients and their support systems to ensure effective implementation of treatment plans.
• Monitor client participation and progress in services, advocating for their needs with service providers.
• Facilitate access to community resources and support networks to promote daily living skills and self-determination.
• Coordinate and ensure continuity of care within the human service system.
• Respond to emergency situations, providing assessments and referrals as needed, including facilitating commitments to psychiatric facilities.
• Maintain accurate and up-to-date case records and prepare required reports within designated time frames.
• Adhere to internal and external case management standards, ensuring compliance with all requirements.
