Job summary

Glade Run Lutheran Services in Butler, PA is seeking a Blended Case Manager to provide client-centered case management for adults and children with mental health needs, linking services and advocating for access to resources in home, school, and community settings.

The position requires a 4-year degree (or RN) with 1–3 years in mental health, a willingness to travel within Butler County, and a caseload around 30. Benefits include mileage reimbursement and comprehensive coverage.

Qualifications

• Bachelor's degree in sociology, social welfare, psychology, or related field, RN option, or related experience.
• 1–3 years case management or mental health experience preferred.
• Valid driver's license and willingness to travel within the county.

Responsibilities

• Obtain service history and complete assessment and service plans accurately.
• Assist clients in accessing community services and essential needs.
• Provide on-call services and develop crisis plans.
• Maintain professional relationships with community partners and staff.
• Document per regulations and maintain EMR entries.
• Monitor and discharge clients with timely documentation.
