Job summary

Community Guidance Center in Pennsylvania seeks a Blended Case Manager to support up to 30 consumers. Responsibilities include developing goals, coordinating services, and monitoring progress within the Service Plan and regulatory guidelines.

Requirements include a bachelor’s degree in a related field or RN, plus clearances, strong organizational and communication skills, and the ability to work under stress. Full-time role with benefits and travel as needed.

Qualifications

• Bachelor’s degree in sociology, social work, psychology, or related field or RN.
• Act 33/34 Clearances, FBI clearance and mandated reporter training.
• Strong computer and office software skills; able to document and report.
• Excellent communication, organizational and time-management abilities.

Responsibilities

• Assessment and Service Planning with consumer goals aligned to needs.
• Linking with services and helping access resources per plan.
• Monitor service delivery and advocate for consumer needs.
• Resolve problems and coordinate with providers.
• Build informal support networks with family involvement.
• Use community resources and ensure proper documentation.
