Position Summary
Provides case management services to eligible consumers as defined by DPW Title 55 Code Chapter 5240. CCR’s mission is connecting people to services. In this role you will work with a local team to provide the community with care and support that improves mental health and well‑being.
Essential Functions
- The Blended Case Manager provides case management services to adolescents, children, and adults with serious mental illness or emotional disturbance.
- Works with clients and families to access, link, coordinate, and monitor needed services and provides support and advocacy to assist clients in maintaining independence in their home or community.
- Completes all reports and documentation in a timely fashion and maintains quality standards.
- Participates in an on‑call rotation to ensure 24‑hour coverage for holidays, weekends, and daily between 4:30 pm and 8:30 am.
- Manages time effectively to meet the needs of consumers and program requirements, including monthly utilization and regulated documentation completion.
- Is well organized and has excellent verbal and written communication skills.
- Has computer competency and ability to learn agency programs.
- Performs other duties as assigned to achieve the agency’s mission.
- Is available to meet with consumers during normal business hours, evenings, weekends, and during scheduled crisis events. An on‑call BCM is also available via the on‑call hotline 24 hours a day, 7 days a week.
Qualifications
- Bachelor’s degree in Human Services or a related field.
- One year of experience in mental health case management or the mental health field.
- Knowledge of case management principles, computer literacy, familiarity with the Recovery Model, and knowledge of Schuylkill and Northumberland County resources.
- Valid driver’s license, proof of auto insurance, Act 33/34 and FBI clearances required.
- Act 33/34 and FBI clearances.
- Car insurance coverage verification.
- Able to exercise sound judgment, flexible scheduling, initiative, cooperation, empathy, and maintain strict confidentiality.
