Qualifications

• Bachelor’s degree in a human services field or an Associate with 12 relevant credits and 2 years’ mental health/human services experience.
• Valid driver’s license and ability to travel within the service area.
• Ability to access community resources and coordinate services.
• Must be able to participate in on-call crisis coverage as required.

Responsibilities

• Service Plan Development and updates based on client needs.
• Adherence to charting requirements and documentation.
• Benefit acquisition and maintenance for client services.
• Participates in agency investigations as needed.
