Job summary

RHA Health Services, LLC is hiring a Blended Case Manager to build strong networks of community resources and advocate for clients across mental health, intellectual and developmental disabilities services. The BCM links clients with needed services, helps navigate complex systems, and provides crisis support on-call 24/7.

The role requires a degree in human services or related field and a valid driver’s license.

Qualifications

• Bachelor’s degree in human services field OR Associate’s degree with 12 relevant credits and 2 years’ experience in mental health/human services.
• Valid driver’s license.
• Strong communication and advocacy skills.

Responsibilities

• Develops service plans, treatment plans, and crisis plans based on client needs.
• Prepares documentation including assessments, admissions, medication records, progress notes, and plans within required timeframes.
• Assists clients in gaining funding for needed services and entitlements.
• Participates in agency investigations and performs related duties as assigned.
