Job summary

RHA Health Services is hiring a Blended Case Manager to support individuals with intellectual/developmental disabilities, mental health, and substance use needs. You will develop service plans, link clients to community resources, and advocate for access to services.

The role requires a bachelor’s degree in a human services field or an associate’s with relevant credits and experience. On-call coverage is available 24/7 to assist clients as needed.

Qualifications

• Bachelor’s degree in human services-related field (e.g., sociology, social work, psychology, gerontology, anthropology, political science, history, criminal justice, theology, nursing, counseling or education)
• Associate’s Degree with 12 College Credits relevant to position and two years’ experience in mental health/human services

Responsibilities

• Develops service plans, treatment plans and crisis plans based on client strengths and needs
• Gathers client data across personal, familial, social, vocational, and cultural domains
• Maintains documentation: assessments, admissions, medication records, progress notes, incidents, service and crisis plans
