Job summary

RHA Health Services, LLC seeks a Blended Case Manager to support children and adults with behavioral health and developmental needs. You will link clients to community resources and advocate within complex systems to secure housing, education, and medical access.

You’ll develop service and crisis plans, coordinate services, and document progress while working with families and multiple agencies. On-call crisis coverage is provided 24/7, with collaboration across the treatment team.

Qualifications

• Bachelor’s degree in human services related field or Associates degree with 12 credits and 2 years in the mental health/human service field.
• Strong verbal and written communication skills.
• Ability to access community resources and coordinate services.

Responsibilities

• Develops service plans and crisis plans based on client needs.
• Maintains documentation, assessments, and progress notes within required timeframes.
• Gains funding authorization for needed services and entitlements.
• Collaborates with county and other providers to assess client needs.
