Job summary

RHA Health Services is seeking a Blended Case Manager to support individuals with intellectual and developmental disabilities and mental health or substance use challenges. The role emphasizes developing service plans, coordinating resources, and maintaining documentation.

You will work with the treatment team, families, and community providers to ensure appropriate services and crisis planning, with a focus on accessibility and quality care.

Qualifications

• Bachelor’s degree in human services related field OR Associates Degree with 12 credits and two years’ mental health/human services experience.
• Valid driver’s license.
• Must be able to access community resources and services.
• Excellent verbal and written communication skills.
• Ability to collaborate with agencies and teams.

Responsibilities

• Develops service plans, treatment plans, and crisis plans based on client data.
• Prepares documentation including assessments, admissions, and progress notes.
• Assists client in obtaining funding authorization for services.
• Participates in agency investigations.
• Performs other related duties as assigned.
