Job summary

Blair Family Solutions is seeking a Case Manager to coordinate services for individuals with mental health needs. You will assess clients, develop Service Plans, link with medical and social providers, and monitor progress to ensure effective outcomes.

Responsibilities include advocating for services, coordinating care across teams, and preparing discharge plans. A relevant degree or RN license is required, with pay reflecting degree and experience.

Qualifications

• Bachelor’s degree in related social sciences or nursing; or RN license.
• Associates with relevant field may apply; pay varies by degree/experience.
• Licensure as applicable (RN or other professional license) is preferred.

Responsibilities

• Complete client assessments to determine service needs.
• Develop a Service Plan based on assessment findings.
• Help clients obtain needed services and link with providers.
• Monitor and follow up to ensure plan is effectively implemented.
• Coordinate and provide client-centered care; advocate when needed.
• Support wellness and autonomy; maintain progress records and communications.
• Prepare discharge plans and coordinate post-discharge requirements.
