Job summary

SCAN Health Insurance is seeking a Registered Nurse (RN) – Care Management to deliver high-touch, person-centered care to members with complex medical needs. You will conduct in-home and telephonic assessments, create and manage care plans, educate members and caregivers, and collaborate with an interdisciplinary team to stabilize risk and improve outcomes.

Qualifications

• Associate’s degree in nursing (ASN) required.
• Bachelor’s degree in nursing (BSN) preferred.
• CA Registered Nurse (RN) required.
• CCM certification strongly preferred.
• BILINGUAL English/Spanish required.
• Travel 50%+ in the assigned area with valid license and insurance.

Responsibilities

• Conduct in-home and telephonic assessments to develop individualized care plans.
• Educate members and caregivers on disease processes and self-management.
• Coordinate with interdisciplinary care team to improve health outcomes.
• Ensure medication safety and reconciliation with providers.
• Document care plans and progress across multiple systems.
