Job summary

Tri-Counties Regional Center in Oxnard is seeking a Service Coordinator II (Bilingual Required) to provide person-centered consultation and coordination for individuals with developmental disabilities.

You will conduct IFSP and IPP planning, participate in IDT staffings, and connect clients with community resources and funded services to support independent living and community integration.

Qualifications

• Bachelor’s degree in a Human Services or related field.
• 2 years social and/or mental health experience preferred.
• Valid California driver’s license and transportation or acceptable substitute.

Responsibilities

• Provide person-centered consultation, advocacy, and service coordination.
• Maintain regular contact and develop IFSP/IPP for individuals served.
• Participate in IDT staffings and community meetings (IEP, day program).
• Coordinate and secure funded services (behavior, supported living, day programs).
