Job summary

TCRC is seeking a Service Coordinator II Enhanced Coordination in Oxnard. This role focuses on person-centered consultation, advocacy, and coordination for individuals with developmental disabilities, working with a multidisciplinary team to implement IPP and ensure access to community resources.

The position requires bilingual English/Spanish proficiency, a Master’s or Bachelor’s degree in a related field, and experience in mental health or social work.

Qualifications

• Bilingual English/Spanish required.
• Master’s or Bachelor’s degree in human services or related area.
• 2 years of applicable experience in mental health or social work preferred.

Responsibilities

• Manage a caseload with a 1:40 ratio.
• Develop and monitor IPP/PP planning and progress reports.
• Collaborate with PCP Specialist, Cultural Specialist, and Self-Directed Services team.
• Advocate for clients and coordinate services with community agencies.
