Job summary

Tri-Counties Regional Center is seeking a Service Coordinator II to provide person-centered consultation, advocacy, and service coordination for individuals with developmental disabilities in the Santa Barbara area.

The role involves interdisciplinary team participation, development of service plans, and coordination of funded services to support daily living and community integration. A Bachelor’s degree and 2+ years in related fields are required.

Qualifications

• Bachelor’s degree in a Human Services or related field.
• A minimum of 2 years of social and/or mental health experience.
• Valid California driver’s license and transportation or acceptable substitute.
• Experience with developmental disabilities preferred but not required.

Responsibilities

• Conduct strength-based assessments and involve individuals and families in planning.
• Participate in IDT staffings and community meetings (IEP, day program).
• Provide information and referrals to community services and resources.
• Coordinate and secure TCRC funded services (behavior, housing, day programs, equipment).
• Develop and maintain relationships with vendors and community agencies.
