Job summary

Tri-Counties Regional Center in Santa Barbara, CA is seeking a Service Coordinator 2 to provide person-centered consultation, advocacy, resource information and service coordination for people with developmental disabilities who require more involved and specialized assistance.

The role involves collaborating with families, attending IEP and IDT meetings, connecting clients to community resources, coordinating funded services, and promoting independence through strength-based planning.

Qualifications

• Master's or Bachelor's degree in human services or related field.
• 2 years of case management experience.
• 1 year experience with individuals with developmental disabilities preferred.
• Valid California driver's license with reliable vehicle.
• Bilingual in Spanish and English.
• Willing to travel to any location.

Responsibilities

• Maintain regular contact and develop IFSP and IPP.
• Participate in IDT staffings and community meetings.
• Provide information and referrals to community resources.
• Coordinate and secure TCRC funded services.
• Conduct strength-based assessments.
• Engage families in person-centered planning.
