Job summary

Kern Regional Center in California is seeking a dedicated Service Coordinator/Case Manager to coordinate services, advocate for consumers, and arrange supports for individuals with developmental disabilities and their families.

You will develop IPP/PCP plans, prepare progress reports and annual reviews, and connect families with community resources. Must have a BA/BS and bilingual Spanish/English, plus strong organizational and communication skills.

Qualifications

• BA/BS degree in human services or related field; two years of experience with the developmentally disabled.
• Bilingual Spanish/English required.
• Detail-oriented and highly organized.
• Strong computer skills with word processing, spreadsheets, presentations, and industry-specific software.
• Excellent written and verbal communication; timely customer service.

Responsibilities

• Coordinate services and advocate for consumers and families.
• Develop and implement IPP and PCP plans.
• Prepare progress reports and annual reviews.
• Conduct follow-ups and reassessments as needed.
• Identify community resources and referrals.
• Coordinate vendor services and monitor delivery.
• Stay current with state and federal regulations affecting services.
