Job summary

Kern Regional Center is seeking a dedicated Service Coordinator/Case Manager to coordinate services, advocate for consumers and families, and implement IPP/PCP plans. The role involves progress reporting, annual reviews, and resource referrals within California's developmental services framework.

The position requires a BA/BS in a human services field and bilingual Spanish/English, with strong organizational and communication skills, plus a valid driver’s license and reliable transportation.

Qualifications

• BA/BS degree in human services or related field and two years’ experience with the developmentally disabled.
• Bilingual Spanish/English required.
• Detail-oriented and highly organized.
• Strong computer skills with word processing, spreadsheets and presentations.
• Excellent written and verbal communication.
• Provide timely, respectful customer service in person, on the phone, by email or in writing.
• Valid driver’s license, DMV printout, and reliable transportation.

Responsibilities

• Coordinate overall service coordination, advocacy and service arrangements for consumers and families.
• Develop and implement the consumer IPP and PCP plans and progress reports.
• Prepare annual reviews and progress reports; monitor progress and program effectiveness.
• Initiate follow-ups, reassessments and referrals; seek community resources.
• Monitor vendored services and ensure compliance with state/federal regulations and agency policies.
