Job summary

Tri-Counties Regional Center in Santa Barbara is seeking a Service Coordinator II to provide person-centered consultation, advocacy, and coordination for individuals with developmental disabilities who require intensive support. You will participate in IDT meetings, develop IFSP/IPP, connect clients with community resources, and coordinate funded services such as behavior services and day programs.

A Bachelor's degree in Human Services is required along with at least two years of related

• Awareness of entitlement and benefit programs and of public benefit programs.
• Problem-solving ability.
• Ability to make verbal and written presentations to groups.
• Organizational and time management skills.
• Ability to work independently and within a team.

• Understand and adhere to applicable laws, regulations, policies and procedures.

Responsibilities

• Support individuals by maintaining regular contact and doing IFSPs and IPPs.
• Participate in IDT staffings and attend meetings in the community.
• Provide information and referrals to community services and resources.
• Coordinate and secure funded services, including behavior services and respite.
• Develop and maintain networks with vendors and community agencies.
