Job summary

The Providence Community Health Centers, Inc. is seeking an experienced RN for part-time call center triage in Providence, RI. You will triage patient calls, assess needs using protocols, and coordinate with providers to arrange care.

Ideal candidates hold Rhode Island RN licensure, have 5+ years in ER/urgent care, and can communicate effectively, including Spanish speakers. Community health experience is a plus; Bachelor's degree preferred. On-site role.

Qualifications

• Licensed RN in Rhode Island in good standing.
• Minimum of 5 years nursing in ER, primary care, or urgent care.
• Community health nurse experience preferred.
• Strong assessment, critical thinking, and communication skills.
• Experience in triage or rapid nursing process.
• Spanish speaking strongly preferred.
• Bachelor's degree preferred or equivalent work experience.

Responsibilities

• Receive, triage and respond to incoming calls from patients seeking clinical direction and advice.
• Uses standardized adult and pediatric protocols; seeks assistance when protocol not available or appropriate.
• Assesss patients' needs by listening and asking proactive questions.
• Directs patient to appropriate level of care and schedules with a PCHC provider when needed.
• Identifies emergent situations and triages to higher level of care.
• Makes clinically appropriate recommendations for action and issue resolution.
