Job summary

Ideal candidates have a Rhode Island RN license in good standing and 3–5 years of nursing experience, with proficiency in EMR and Microsoft Office. The role is based in Middletown, RI, 8a–4p, on-site.

Qualifications

• Licensure as Registered Nurse in Rhode Island by the RI Board of Nursing.
• Graduate of an accredited school of nursing.
• Current Rhode Island RN license in good standing.
• 3-5 years of appropriate nursing experience.
• Proficient in EMR platforms and Microsoft Office (Outlook, Excel, Word).
• Ability to work both on a team and independently; strong communication.

Responsibilities

• Reviews and triages telephone encounters and messages from patients.
• Prioritizes urgent medical triage for patients.
• Provides timely assessment for scheduling into clinician availability.
• Reviews lab & imaging results and directs to clinicians as needed.
• Communicates with care teams and providers with recommendations.
• Develops professional relationships with health care providers and facilities.
• Coordinates team-based care in the absence of the Program Manager.
• Complies with HIPAA and protects patient privacy.
