Job summary

Charles River Community Health is seeking a Call Center Triage Nurse to join the remote care team. You will assess symptoms by phone and in person, provide triage guidance, and support care coordination.

The role requires Massachusetts RN licensure, a BSN/ADN with 5+ years of triage experience in outpatient settings, and 3+ years in ambulatory care. Bilingual abilities are highly preferred, with onsite training as needed.

Qualifications

• Massachusetts nursing license (RN) required.
• BSN or ADN with 5+ years of triage experience in outpatient setting required.
• 3+ years ambulatory/Family Practice triage experience preferred.

Responsibilities

• Assess patients by phone and in person; provide home treatment guidance.
• Gather history and triage information to determine care path.
• Relay triage information between provider and patient; determine disposition.
• Assist in selecting appropriate provider disposition and care utilization.
• Provide telephone advice per guidelines; monitor home treatment effectiveness.
• Develop or update telephone triage guidelines as needed.
• Document all encounters in the medical record.
• Refer patients to emergency or other health center services as indicated.
• Coordinate with Referral Department and Managed Care for follow-ups.
