Qualifikationen

• Active RN licensure in the applicable state.
• BLS Certification.
• EMR experience.
• US work authorization.
• Ability to assess patients without face-to-face interaction and strong communication.

Aufgaben

• Provide competent nursing care by displaying proficiency in this role and executing job responsibilities in a safe and consistent manner.
• Respond to incoming telephonic requests in a dependable manner and exceed expectations.
• Provide triage and symptom management to patients may be remote or in person.
• Utilize standardized protocols for medication management, prescriptions, refills and prior authorizations.
• Review patient medications and update as needed.
• Educate patients and provide direct patient care, including chronic conditions.
• Collaborate with care team to implement and progress care plans.
• Build relationships with patients and caregivers to support preventative care.
• Create a welcoming environment and delegate tasks to team members.
• Participate in care team meetings and clinic operations.
• Deliver a high-quality patient experience with service and respect.
• Perform point of care testing and specimen collection as needed.
