Job summary

Oak Street Health in New York is seeking a Nurse Practitioner (Spanish required) to deliver value-based primary care to adults on Medicare. You will see patients independently within an outpatient clinic as part of a collaborative care team with physicians, nurses, and care managers.

Responsibilities include annual wellness visits, care coordination, patient education, and home visits as needed. Candidates must have an NP degree, national certification, active license, DEA, and Spanish bilingual

Qualifications

• MSN, DNP, or NP certificate program graduate.
• National certification in one of: Family NP, Adult-Gerontology Primary Care NP, Adult NP, Gerontological NP.

Responsibilities

• Provision of exceptional primary care.
• Conduct office visits for routine and acute issues.
• Annual wellness visits and HRAs with thorough documentation.
• Care coordination with other providers and facilities.
• Population health leadership with the care team.
• Phone triage and outreach support.
• Educating patients on health conditions and treatments.
• Participating in Oak Street Health promotional activities.
• Conducting home visits as needed.
