Job summary

Oak Street Health in Chicago is seeking a Registered Nurse to join our Center Nursing team. The Center Nurse participates in a value-based care model, performing assessments, managing chronic conditions, and collaborating with an interdisciplinary care team to reduce hospitalizations.

You will administer medications and vaccines, provide patient and caregiver education, conduct on-site testing, and ensure timely documentation.

Qualifications

• Active, unrestricted Registered Nurse licensure in the state that you reside.
• BLS or CPR certification.
• Fluent written and spoken English language skills.
• US work authorization.
• Electronic Medical Record (EMR) experience.
• Ability to maintain patient confidentiality and process information confidentially.

Responsibilities

• Support patient assessment and prioritization for triage to ensure appropriate care and avoid unnecessary hospital visits.
• Perform protocol-driven chronic condition management.
• Perform skilled nursing procedures including wound care and medication administration (including vaccines).
• Provide patient and caregiver education regarding chronic conditions and prevention in person, over the phone, or in group settings.
• Conduct medication reviews within nurse-led visits to ensure patient safety and adherence.
• Conduct onsite testing (including point-of-care labs, EKGs, specimen collection) and use patient data to identify and close gaps in preventative screenings.
• Review and act upon high-priority abnormal laboratory results to maintain patient safety and continuity of care.
• Coordinate with providers and the care team to manage complex care plans and transitions from hospital to clinic.
• Utilize standardized protocols for medication management, renewals and prior authorizations.
• Ensure clinical communications are managed and patient visits are closed timely.
• Delegate to medical assistants and phlebotomists to support patient flow.
• Perform other related duties as assigned.
