Oak Street Health in Bronx, NY is seeking a Nurse Practitioner to provide effective, value-based primary care to adult patients enrolled in Medicare. You will work within a collaborative care team to deliver longitudinal care and build trust with patients, guiding care plans and outcomes.

The role requires a MSN/DNP/NP certificate, an active NP license, a valid DEA, US work authorization, and Spanish bilingual proficiency. 40-hour weeks, full-time, with a competitive benefits package.

Qualifications

• Master of Science in Nursing, Doctor of Nursing Practice, or NP Certificate Program graduate.
• Active, non-probationary state NP license.
• Active DEA license.
• US Work Authorization.
• Bilingual proficiency in Spanish.

Responsibilities

• Provision of exceptional primary care.
• Conduct office visits for routine and acute issues.
• Administer Annual wellness visits and health risk assessments with thorough documentation.
• Care coordination with other providers, specialists, testing facilities, and agencies.
• Population health leadership with the Care Team to ensure preventive care.
• Assisting the care team with phone triage and outreach.
• Educating patients on health conditions, care plans, and treatments.
• Participating in Oak Street Health promotional activities.
• Conducting home visits as needed.
• Other duties, as assigned.
