Job summary

Oak Street Health is seeking an RN, Case Manager to lead care coordination for complex patients at our Bronx and Queens-area clinics. The role focuses on preventing avoidable admissions, optimizing resource use, and coordinating care with PCPs, nurses, social workers, and other team members.

It is an on-site, field-based position with travel across communities served. The ideal candidate holds an RN license, CCM certification, and 6–8 years of nursing experience, with Spanish fluency a plus.

Qualifications

• Spanish fluency required
• Current RN license in assigned state required; Bachelor's degree in nursing preferred
• Certified Case Manager (CCM) required, or willingness to obtain within 12 months of hire

Responsibilities

• Lead care coordination for assigned complex patients in a value-based care setting.
• Drive reductions in admissions, readmissions, and medical utilization.
• Own overall care coordination and be primary point of contact for the care team.
