Job summary

Oak Street Health is seeking a Center Nurse to support clinical care within a value-based care model. You will assist with assessments, chronic condition management, wound care, and medication administration including vaccines.

You’ll educate patients and caregivers, perform onsite testing, and collaborate with the care team to coordinate transitions from hospital to clinic. The role requires RN licensure, BLS/CPR, EMR experience, and strong communication.

Qualifications

• Active, Unrestricted Registered Nurse licensure in the state you reside.
• BLS or CPR certification.
• Fluent written and spoken English language skills.
• US work authorization.
• Electronic Medical Record (EMR) experience.
• Ability to maintain patient confidentiality and process information confidentially.
• Ability to assess patients without face-to-face interaction, strong communication and assessment skills.

Responsibilities

• Support patient assessment and prioritization for walk-in and telephonic triage to ensure appropriate care and avoid unnecessary visits.
• Execute protocol-driven chronic condition management.
• Perform skilled nursing procedures including wound care and vaccines.
• Provide patient and caregiver education regarding chronic conditions and preventive care in person, over the phone, or in group settings.
• Conduct medication reviews within nurse-led visits to ensure patient safety and adherence.
• Conduct onsite testing (point-of-care labs, EKGs, specimen collection) and use data to close gaps in preventive screenings.
