Oak Street Health is seeking a Center Nurse to join a multidisciplinary team delivering value-based care. The Center Nurse conducts assessments, manages chronic conditions, administers medications and vaccines, and guides patients and families with education to improve outcomes.

You will work within a supportive care team and report to the Practice Manager. Responsibilities include triage, care planning, on-site testing, and coordination of transitions from hospital to clinic.

Qualifications

• Active, unrestricted RN license in your state.
• US work authorization.
• BLS or CPR certification.
• Fluent written and spoken English.

Responsabilités

• Support patient assessment and triage to ensure appropriate level of care.
• Execute protocol-driven chronic condition management.
• Perform wound care and medication administration (including vaccines).
• Provide education on chronic conditions and preventive care.
• Review medications and ensure patient safety and adherence.
• Conduct onsite testing (POC labs, EKGs, specimen collection).
• Collaborate with providers to manage complex care plans.
