Job summary

Oak Street Health seeks a Registered Nurse to join the Center team in a Value-Based Care model. You will support patient assessments, chronic condition management, and nursing procedures, including vaccines, to promote long-term wellness.

Responsibilities include onsite testing, medication reviews, education for patients and caregivers, and collaboration with providers to coordinate care plans. Strong communication and confidentiality are essential.

Qualifications

• Active, unrestricted RN license in your state.
• BLS/CPR certification required.
• Fluent English (written and spoken).
• US work authorization.
• EMR experience.
• Maintain patient confidentiality.
• 2+ years healthcare experience as an RN.

Responsibilities

• Support patient assessment and triage to ensure appropriate care.
• Execute protocol-driven chronic condition management.
• Perform wound care and medication administration (including vaccines).
• Educate patients and caregivers on chronic conditions and preventive care.
• Conduct medication reviews during nurse visits for safety and adherence.
• Onsite testing (POC labs, EKGs, specimen collection) and data review.
• Coordinate with providers to manage complex care plans.
• Coordinate weekly transitions from hospital to clinic.
• Use nursing protocols for med management and renewals.
• Documentation and closure of patient visits in a timely manner.
• Delegate to medical assistants and phlebotomists as needed.
