Job summary

National Youth Advocate Program is seeking a Medical Coordinator for La Jornada in Duluth, GA. The role coordinates medical, dental, mental health, and pharmacy services for children in care, including urgent and preventive care, and keeps complete, confidential health records.

You will communicate with care teams, coordinate appointments and interpretation, obtain provider MOUs, and support foster families with medical policy education.

Qualifications

• Licensed Practical or Vocational Nurse (LPN/LVN) or RN with bachelor’s degree or higher.
• Bilingual fluency in English and Spanish.
• Minimum of 1 year in public health or healthcare with care coordination/administration experience.
• 1 year in child welfare in a social service setting.

Responsibilities

• Coordinate medical, dental, mental health, and pharmacy services for children in care.
• Document services and maintain confidential health records.
• Coordinate appointments and interpretation services; obtain MOUs with providers.
• Support foster parents with medical policy education and required responsibilities.
• Assist with audits and program leadership support.
