Responsibilities include coordinating care, scheduling appointments, documenting with health record systems, supporting case managers, and collaborating with cross-functional teams to ensure timely and appropriate medical services.

Qualifications

• RN license required.
• Bachelor's or higher in a health-related field.
• Bilingual English/Spanish required.
• Experience in care coordination or public health preferred.

Responsibilities

• Oversee health services for children in care.
• Supervise Medical Coordinators and provide staff training on medical topics.
• Develop and maintain policies for confidential health records and PHI compliance.
• Coordinate health care appointments and assist with transportation and documentation.
