Job summary

National Youth Advocate Program seeks a Medical Coordinator for La Jornada to arrange and coordinate medical, dental, mental health, and pharmacy services for children in care. You will document services, coordinate care with cross-functional teams, and ensure privacy and compliance with PHI guidelines.

You will support intake/discharge processes, prepare TARs, and assist families with medical responsibilities, while maintaining records and communicating with care teams.

Qualifications

• LPN/LVN or RN with a bachelor’s degree or higher health qualification.
• Bilingual fluency in English and Spanish is required.
• Experience in public health, health administration, or care coordination preferred.

Responsibilities

• Coordinate medical, dental, mental health and pharmacy services for children in care.
• Maintain confidential health records and ensure PHI compliance.
• Arrange appointments, complete TARs, and support medical staff during visits.
• Educate foster parents on medical policies and required forms.
• Maintain medical supplies and ensure documentation uploads to records systems.
