Phamily is seeking a Part-time Patient Support Specialist in the Indianapolis area. The role is remote initially and will transition to onsite as the new office opens.

You will conduct high-volume patient outreach, educate on care programs, and support enrollment with empathetic phone-based engagement. Strong bilingual communication (English/Spanish), care management experience, and a commitment to accurate documentation are essential.

Qualifications

• Bilingual English and Spanish is required.
• Prior care management experience is preferred.
• Excellent written and verbal communication, especially by phone.
• Ability to perform high-volume patient outreach professionally.
• Call center experience is a plus.

Responsibilities

• Conduct high-volume inbound and outbound patient outreach via phone to educate, engage, support, and enroll eligible patients.
• Explain the purpose, benefits, and value of care management programs clearly to patients.
• Build rapport with patients and address questions or objections to encourage participation.
• Manage the Scheduling inbox and route clinical concerns with timely follow-up.
• Accurately document patient interactions, outreach attempts, and enrollment outcomes.
• Collaborate with client care teams to ensure a seamless patient experience.
• Follow outreach workflows, scripts, and compliance requirements while adapting conversations.
• Identify and elevate patient concerns and contribute to process improvements.
