Amazing Home Care Inc. in the Bronx and Queens is seeking a dedicated Bilingual Patient Intake Coordinator (Cantonese/Mandarin) to streamline enrollment for home care services.

You will manage eligibility reviews, handle 40-50 cases, conduct calls to generate leads, gather documents, and collaborate with clinicians, caregivers, and clients to ensure smooth onboarding. Strong organizational, communication, and data-entry skills are required; Salesforce experience is a plus.

Qualifications

• Fluency in Cantonese and Mandarin (written and verbal) required.
• Experience in Intake or Onboarding home care clients preferred.
• Strong organizational skills and attention to detail.
• Excellent communication and interpersonal skills.
• Ability to work independently and as part of a team.
• Proficiency in basic computer applications; Salesforce a plus.

Responsibilities

• Explain program benefits and conduct initial eligibility reviews (Medicaid/MLT C) for home care enrollment.
• Conduct outbound calls to 40-50 active patient cases and convert 30+ monthly conversations into qualified leads.
• Coordinate and manage the intake process, gathering necessary documentation.
• Collaborate with healthcare professionals, caregivers, and clients to ensure smooth onboarding and enrollment.
• Maintain accurate, confidential records of all intake activities and participant information.
• Provide exceptional customer service by educating clients on program guidelines and procedures.
