You will manage eligibility reviews, handle 40-50 cases, conduct calls to generate leads, gather documents, and collaborate with clinicians, caregivers, and clients to ensure smooth onboarding. Strong organizational, communication, and data-entry skills are required; Salesforce experience is a plus.

Qualifications

• Fluency in Cantonese and Mandarin (written and verbal) required.
• Experience in Intake or Onboarding home care clients preferred.
• Strong organizational skills and attention to detail.
• Excellent communication and interpersonal skills.
• Ability to work independently and as part of a team.
• Proficiency in basic computer applications; Salesforce a plus.

We are currently seeking a dedicated and Bilingual (Cantonese/Mandarin) Patient Intake Coordinator to join our team at our Bronx and Queens location. As a Patient Intake Coordinator, you will play a crucial role in facilitating the enrollment process for individuals seeking home care services.

Responsibilities:

• Explain program benefits, and conduct initial eligibility (Medicaid and Managed Long-Term Care) reviews and screenings for individuals interested in participating for home care services.
• Conduct outbound calls to an assigned census of 40-50 active patient cases and answer incoming calls with a goal of 30+ monthly conversations converted into qualified leads.
• Coordinate and manage the intake process, including gathering necessary documentation and information.
• Collaborate with healthcare professionals, caregivers, and clients to ensure smooth onboarding and enrollment procedures.
• Maintain accurate and confidential records of all intake activities and participant information.
• Provide exceptional customer service by educating clients and caregivers on program guidelines and procedures and addressing inquiries, and concerns, throughout the intake process.

Requirements:

• Fluency in both Cantonese and Mandarin (written and verbal) is essential.
• Experience in a similar role in Intake or Onboarding home care clients.
• Strong organizational skills with attention to detail.
• Excellent communication and interpersonal skills.
• Ability to work independently and collaboratively in a team-oriented environment.
