Qualifications

• At least 1 year of experience in care coordination, case management, patient services, clinical support, or a related healthcare setting.
• Fluent in Spanish and English required.
• Home care experience preferred.

Responsibilities

• Build positive relationships with patients and families as a reliable contact.
• Coordinate home care services according to each patient’s needs and care plan.
• Assist with scheduling, follow-ups, and in-home assessments.
• Communicate care plans, services, and expectations clearly to patients and caregivers.
• Follow up on patient needs and changes in their care situation.
• Maintain accurate and timely patient records and documentation.
• Coordinate with healthcare providers, home health aides, and other care-team members.
• Ensure services are delivered per agency policies, regulatory requirements, and quality standards.
• Respond promptly and professionally to patient and family concerns.
