Personal Touch Home Care in Brooklyn, NY is hiring an Intake Specialist to handle initial intake information for patients seeking home care services. This is a fully in-person role based out of our Brooklyn office.

The ideal candidate has at least 1 year of home care experience; bilingual English-Spanish or English-Creole is preferred. Strong data entry, communication, and organizational skills are essential to coordinate referrals, admissions, and enrollment.

Qualifications

• Minimum 1 year experience in home care
• 18 years of age or older; HS diploma or GED
• Bilingual English-Spanish or English-Creole preferred
• Proficient with Microsoft Office and data entry
• Knowledge of Medicaid/Medicare concepts and long-term care insurance

Responsabilités

• Receives and processes referrals from various sources
• Answers phones and gathers information for admissions
• Inputs data and maintains notes in referral system
• Maintains organized intake records and verifies insurances
• Communicates with patients and families through enrollment and start of cares
• Coordinates with contract agencies for authorizations and staffing
