Job summary

Amazing Home Care Inc. is seeking a dedicated Bilingual Patient Intake Coordinator for our Bronx location. You will facilitate enrollment for individuals seeking home care services and manage an intake workflow from documentation through onboarding.

The role requires fluency in English and Spanish, experience in intake for home care, strong organization and communication skills, and the ability to work independently plus collaboratively. Health insurance and growth opportunities are included.

Qualifications

• Fluency in English and Spanish (written and spoken).
• Experience in intake or onboarding for home care clients.
• Strong organizational and interpersonal skills.
• Proficiency with basic computer applications; Salesforce a plus.

Responsibilities

• Explain program benefits and conduct eligibility reviews for Medicaid and managed long-term care.
• Handle 40–50 patient intake cases and convert 30+ monthly calls to qualified leads.
• Coordinate intake process and gather necessary documentation.
• Collaborate with healthcare professionals, caregivers and clients to onboard smoothly.
• Maintain confidential records of all intake activities and participant information.
• Provide excellent customer service and educate clients on program guidelines throughout intake.
